Key Responsibility

• Handle and assess inquiries concerning drug dosages, contents, regimen reviews, interactions and contraindications.
• Monitor, control, and assess inventory levels to ensure sufficient stock availability while minimizing wastage.
• Screen, interpret, prepare and check all prescriptions for accuracy, appropriateness, and completeness before dispensing.
• Responsible for preparing, reviewing, and analyzing reports such as incident reports and other relevant pharmacy service reports for presentation and discussion at the Hospital Medical Advisory Committee (HMAC) Meeting as well as the Group Medical Advisory Committee (GMAC) Meeting.
• Perform and validate medication appropriateness reviews and medication reconciliation as needed.
• Ensure the preparation and dispensing of extemporaneous products; prepare formulations when necessary, according to standard formulas.
• Adhere and enforce the “5 Rights” of medication preparation and dispensing.
• Participate in and oversee drug inventory activities, including purchasing, receiving, issuing, and storage.
• Ensure the handling of Dangerous Drugs and Psychotropic Substances in compliance with MOH laws and hospital policies.
• Provide and conduct in-service training sessions for healthcare workers and patient counselling sessions when necessary.
• Prepare and verify pharmacy staff rosters, leaves, and replacements, ensuring sufficient manpower coverage.
• Document, analyze, and report all medication errors, near misses, and adverse drug reactions in a timely manner.
• Ensure and review safe disposal of expired and damaged medications according to policy.
• Liaise and report to the Chief Pharmacist and relevant committees on risk management and medication safety issues.
• Gather, collate, and submit operational data, reports, and statistics accurately and on time.
• Liaise with the Chief Pharmacist and management on complex or high-impact decisions.
• Control and check all records related to dispensing, stock movement, and controlled drug registers.
• Liaise and coordinate with doctors, nurses, and other healthcare professionals to ensure continuity of medication supply and patient care.
• Source and procure medications during emergencies or shortages.
• Coordinate and assist in hospital committees and pharmacy clinical service activities.
• Perform and support any other duties assigned by the Chief Pharmacist.

Job Requirement & Education

• Must possess at least Bachelor Degree/ Masters in Pharmacy
• Registered with Pharmaceutical Board
• Able to communicate effectively and work independently
• Minimum 5 years of working experience in a hospital pharmacy setting
• Good knowledge of hospital pharmacy operations and medication safety
• Strong communication and teamwork skills
• Able to work independently and under pressure
